PDF icon ACC W9 The W-9 form is used to officially ask a person or business to provide their name, address, and taxpayer identification number so that the requesting party can properly issue tax documents to the IRS.
excel icon ACC 60 The ACC 60 is a the form that clubs and organizations use to access money in their SGA Account.  The form should be completed fully, and include the original receipts and signature from the club advisor.
microsoft excel icon ACC 61 The ACC 61 is a the form that clubs and organizations use to access money in their Club Account.  The form should be completed fully, and include the original receipts and signature from the club advisor.
PDF icon SGA Club Report Form This forms should be utilized for clubs to submit their reports to our Secretary before our monthly meetings.
microsoft word icon ACC 103 The Services Contract is used any time a campus organization wishes to contract services from an outside vendor / speaker.
microsoft word icon SGA Motion Form Any time a new motion is brought forth for discussion in SGA, a motion card must be completed. Motions must include the students name, motion, and have to be properly seconded in order to be discussed.
microsoft word icon Campus Activity Form In order for individuals or organizations to host events / fundraisers on campus, the campus activity form must be completed and submitted to the Student Activities Director for approval.
microsoft word icon Club Deposit Form The Deposit form is utilized by clubs and organizations to deposit money collected / raised into their club account.
