Admissions Checklist 
A quick reference of important steps in the admissions process.

Retest Referral Form
Information and instructions about the retest policy and eligibility options. This form is required for students wanting to retake the placement test. The form must be completed, signed, and submitted to the Placement Testing Office on or before your scheduled retest session.

Score Request form
This form is required if you would like an official copy of your placement test scores sent to another institution. Submit the completed and signed form to the Placement Testing Office and allow one business day to process.
