1. Download the application; save to your computer’s desktop by clicking on File (Main menu-top left of screen).

2. Click “Save Page As.” In the “Save As” screen, select the destination of the application (Desktop).

3. Click “Save” and close the document.

4. Open the document from your Desktop and complete each section.

5. Once you have completed the application, click on File (Main menu).

6. Click “Save.”

The completed application can be saved and printed for your records. Print one copy and forward with your original signature to the Human Resources department.
